By checking this box, I acknowledge and agree to the following:

I understand that participation in fitness programs, classes, and related activities at Berkshire Nautilus involves inherent risks, including the potential for injury, paralysis, or even death. I voluntarily assume all such risks, whether known or unknown, and accept full responsibility for my participation.

I agree to follow all stated terms, conditions, and safety guidelines. If I notice any unusual hazards during my participation, I will remove myself from the activity and notify staff immediately.

I, on behalf of myself, my heirs, and personal representatives, hereby release and hold harmless Berkshire Nautilus, its staff, officers, agents, affiliates, and facility owners from any and all liability, claims, or demands arising from my participation, including those caused by the negligence of the releasees.

If I am signing on behalf of a minor participant, I certify that I am the legal parent/guardian and consent to this agreement on their behalf. I also agree to indemnify and hold harmless Berkshire Nautilus and all releasees for any claims related to the minor’s participation.
